NDIS Local Area Coordination Services

APM Communities delivers NDIS Partners in the Community services in parts of Western Australia, Queensland, and the Northern Territory.

Our Local Area Coordinators (LACs) help people with disability, National Disability Insurance Scheme (NDIS) participants, families and carers to identify and access the support they need. This includes helping you to realise your individual goals and aspirations by connecting you to community and mainstream services.

While we continue to follow advice from the NDIS and Department of Health around the opening of our local offices, if you, or someone you care for, are an NDIS participant in need of LAC services please call us on 1800 276 522 and we will connect you with a member of our team.

They will work with you to find the best way to support and communicate with you.

Local Area Coordinators

We know everyone’s needs are different and we also know how quickly your circumstances can change or become more complicated.

Our LACs work with you directly to identify and access the most suitable support services for you. Your LAC will:

  • Meet with you, your family and carer to learn about your current situation
  • Make it a priority to understand your goals
  • Develop a plan to help you achieve your goals
  • Link you to information sources, supports and services in the community
  • As required, help you connect with NDIA to determine your eligibility for the scheme

If you or someone you care for needs help accessing the NDIS or other support services in your community, we can guide you through the process.

The NDIA will contact you by letter to confirm your eligibility for the scheme.

How a cricket dream came alive for Raz

NDIS Participant Raz lights up with pride as he describes how finally being actively involved in the sport he loves makes him feel alive.

Raz now plays cricket with a local WA club, St Marys, thanks to a collaboration between APM Communities and the Western Australia Cricket Association (WACA).

The team launched a 'come and try' short indoor cricket series to provide the opportunity to participate in an integrated modified cricket game designed by the WACA.

It was the perfect opportunity for Raz and other cricket fans to access the sport.

APM Communities currently support people with disability and NDIS participants in the following areas:

Western Australia: Perth Metro, South West, Great Southern, Inner Wheatbelt

Queensland: Fraser Coast, Cherbourg, North Burnett, South Burnett

Northern Territory:
Darwin, Litchfield, Palmerston

NDIS Events

We often hold free events for people with disability, carers and families to learn more about the NDIS.

Come along to meet some of our LACs, see if you're eligible for the NDIS or access services in the community.

How to get started

Our LACs will help you with understanding what you need for accessing the scheme or developing your NDIS Plan.

An NDIA delegate is responsible for approving the funding attached to your plan. When your plan is approved, your LAC will help you put your plan into action.

Not eligible for the NDIS?

Not every person with a disability will be eligible for an NDIS Plan, if you or the person you care for need help, we can still support you with information and help to connect you to local mainstream and community organisations.
