SERVICE UPDATE

From Tuesday, 24 March, all APM Local Area Coordinators (LACs) will no-longer carry out face-to-face meetings due to the COVID-19 virus.

If you're an NDIS participant in need of Local Area Coordination services please call 1300 276 522 and we will connect you with a member of our team.

They will work with you to find the best way to support and communicate with you.

We are currently following advice from the National Disability Insurance Agency and the Department of Health in regards to the opening of our sites.

In some locations, our APM Communities offices may be closed and our teams can be accessed by calling 1300 276 522.

For non-NDIS related enquiries, please call our APM Customer Service Centre on 1800 276 276.

APM has been selected to deliver National Disability Insurance Scheme (NDIS) Partners in the Community services in parts of Western Australia, Queensland, and Darwin in the Northern Territory.

Western Australia

APM Communities delivers Local Area Coordination services in the following areas:

  • Perth Metro
  • Great Southern
  • South West
  • Inner Wheatbelt

Queensland

APM Communities delivers Local Area Coordination services in the Maryborough service area, which includes:

  • Fraser Coast
  • Cherbourg
  • North Burnett
  • South Burnett

Our Local Area Coordinators (LACs) help people with disability, NDIS participants, families and carers to identify and access the support they need.

This includes helping you to realise your individual goals and aspirations by connecting you to community and mainstream services.

LACs also work with these services and organisations to be more inclusive and supportive of people with disability.

We are proud to provide Local Area Coordination services and our LACs are dedicated to helping you access the right support.

Let’s keep things simple

LAC's work with you directly to identify and access the most suitable support services for you

We know everyone’s needs are different.

We also know how quickly your circumstances can change or become more complicated.

Your LAC will:

  • Meet with you, your family and carer to learn about your current situation

  • Make it a priority to understand your goals

  • Develop a plan to help you achieve your goals

  • Link you to information sources, supports and services in the community

  • As required, help you connect with the NDIA to determine your eligibility for the scheme

If you or someone you care for needs help accessing the National Disability Insurance Scheme (NDIS) or other support services in your community, we can guide you through the process.

The NDIA will contact you by letter to confirm your eligibility for the scheme.

Starting with the NDIS

Our LACs will help you with understanding what you need for accessing the scheme or developing your NDIS Plan.

An NDIA delegate is responsible for approving the funding attached to your plan. When your plan is approved, your LAC will help you put your plan into action.

Not eligible for the NDIS?

Not every person with a disability will be eligible for an NDIS Plan, if you or the person you care for need help, we can still support you with information and help to connect you to local mainstream and community organisations.

APM and the NDIA

The National Disability Insurance Agency (NDIA) has appointed APM to deliver LAC Services as part of the  Partners in the Community Program.

APM will no longer be an NDIS Registered Provider of Supports. APM will ensure it works with the NDIA so that no NDIA participant is disadvantaged during this exit process.

The NDIA deliver the NDIS to support Australians with disability.

At APM, we invite you to provide us with feedback to help us improve our services.

Your comments will be treated confidentially, and all compliments, complaints and suggestions are taken seriously.

If you make a complaint, we will resolve your issue as quickly as possible at the local level.

You can speak directly to your Local Area Coordinator, or if you prefer, speak to APM’s Team Coordinator.

You can make contact with us in the way you feel most comfortable.

If you would prefer to provide feedback to the NDIS, please visit the feedback and complaints page on the NDIS website.

Alternatively you can escalate your concerns to the NDIS Quality and Safeguards Commission.

NT and QLD

The NDIS Quality and Safeguards Commission (NDIS Commission) is a new independent Commonwealth agency established to improve the quality and safety of NDIS supports and services.

The NDIS Commission is now in operation in all states and territories except Western Australia.
